| Description: | Affinity Health Center is a federally-supported Community Health Center, providing comprehensive primary and preventive health care services in areas of high need. Accepts Medicaid, Medicare, Blue Cross/Blue Shield, Select Health, and Cigna. Patients without coverage are charged according to a sliding fee. Health centers provide health care regardless of your ability to pay and even if you have no health insurance. Services: primary care, counseling and mental health, and dental care. Affinity Health Center also provides education, counseling, referral, and direct services to persons with HIV Disease and/or their families. They can help with place to live, medical attention, and emotional support. It is a community-based advocacy organization and offers public education on HIV and on HIV prevention. Free HIV testing is available. |

These services include prenatal care, school physicals, sport physicals, sports medicine, minor surgery, employment physicals and chronic disease management. Many lab tests can be performed in office. Individual and group education and counseling are provided for health issues such as weight management, nutrition, smoking cessation, prenatal care, and breast-feeding. The Center works in conjunction with Fairfield Memorial Hospital to sponsor the Healthy Kids Clinic, which is a program to assist families in helping their children attain a healthy weight. Accepts most Medicaid and Medicare plans as well as major insurances for payment. |
