Description:This is a federally-supported Community Health Center/School-Based Health Center, providing physicals, health education, counseling, and laboratory services. They treat adults and children including EPSDT (Early and Periodic Screening, Diagnoses and Treatment Program) for Medicaid eligible children, and immunizations. They also have a special focus on helping patients manage chronic diseases such as diabetes, hypertension, and asthma. All Community Health Centers accept Medicaid, Medicare and most private insurances and offer a sliding fee discount program for eligible low income families.
