This site provides information in a question and answer format about the three South Carolina laws which address who can make health care decisions if you are unable to do so. These laws are about health care power of attorney, the living will (also known as the Declaration of a Desire for a Natural Death) and the Health Care Consent Act. Forms for living will and health care power of attorney are available in both English and Spanish, in .pdf format. A link to the text of the Adult Health Care Consent Act is also provided.

Do Not Resuscitate (DNR) Form

DHEC Form Number 3462 (Do Not Resuscitate) is a document made to prevent EMS personnel from employing resuscitative measures or any other medical process that would only extend the patient's suffering with no viable medical reason to perform the procedure. The document is presented to the EMS personnel upon their arrival. A physician must request the form by calling the phone number on this Website. Discuss with your doctor whether you should have this form on hand in case of emergency.
