| Description: | This is a federally-supported Community Health Center, providing comprehensive primary and preventive health care services in areas of high need. The insured and uninsured are welcome. A sliding scale of fees, called the "Good Samaritan Plan," is available to help those in financial distress. The Cooperative's goal is to coordinate care with other providers to offer the community 100% access to care. Services: Family Medicine for people 16 and older and behavioral health. |
