Members also include judges, professors of law, and students. Some of the issues NAELA members assist their clients with include, but are not limited to: public benefits, probate and estate planning, guardianship/conservatorship, and health and long-term care planning. - SCDHHS has provided access to legal health forms and applications in PDF format. Here you can find a Health Care Power of Attorney form, a Living Will form, and Medicaid applications in both English and Spanish.
